The Case Manager III is responsible to develop psycho-social assessments and service plans for children and families to ensure child safety and reduce child risk in accordance with the Improved Outcomes for Children (IOC) initiative. Perform case management duties to a caseload of 12-13 (minimum) including advocacy and assistance with entitlements. Participate in Family Team Conferences (FTCs) and complete Family Assessment Service Plans (FASPs) for designated caseload. Complete progress notes both in the Connections system and offline on template forms. Complete monthly casework statistics using the PROMIS system. Participate in various staff meetings.

ESSENTIAL FUNCTIONS:* Carry a caseload of 12-13 families at a minimum.* Meet regularly and consistently with clients as per contract.* Lead groups as per program and clients needs.* Make home and field visits.* Keep all Family Assessment Service Plans (FASPs) and daily logs. Audit ready at all times.* Complete progress notes both in the Connections system and offline on template forms.* Complete monthly casework statistics using the PROMIS system.* Develop and maintain resource information.* Engage families via extensive outreach including home visits. * Participate in professional/agency community activities as assigned.* Participate in rotation of intake responsibilities.* Participate in cross-program service reviews that support the planning and coordination of servicedelivery and service closure.* Coordinate Service delivery to clients.* Assess the needs of the whole consumer/family including their basic needs such as food, clothing, and shelter.* Ensure that referrals are made to appropriate internal and external services as required to meet the consumers/ familys needs.* Follow-up on referrals made on behalf of a consumer/family and respond to all referrals made toyou/your program.

OTHER DUTIES:* Assist in the scheduling of and participate in Family Team Conferences. * Attend weekly supervision.* Participate in training opportunities as assigned.

* MSW from an accredited school of Social Work or MA/MS in a related field.

* Knowledgeable of children and family services and the Administration for Childrens Services.* Group work experience helpful.* Good organizational skills.* Bilingual skills (Spanish) Required* Interest in professional growth - i.e. student supervision.* Basic Computer skills; knowledgeable in Microsoft office.SPECIFICATIONS FOR PHYSICAL REQUIREMENTS

* Ability to work flexible hours (evenings/Saturdays) as needed.* Regularly required to talk, hear, walk, stand, & sit. * Frequently lifts and/or moves up to 10 pounds. * Frequent sitting to write reports and when meeting with clients.* Able to read printed materials and computer screens. * Able to operate a computer keyboard, mouse, & office equipment. * Able to travel to multiple locations by public transportation within the five boroughs for home/field visits.
