Role and Responsibilities
Responsible for on-site supervision, problem solving and advocacy on behalf of program participants
Assist in creating a safe and enrichingenvironment for clients of CWs Day Service Option program by planning for and providing activities that are of interest to and within the capabilities of those served.
Transport clients to events and community activities.
Follow any established behavior plans and document client progress and any other required data.
Participates in required training sessions and maintainscertifications, whichincludeFirst Aid and CPR, PMT,BloodbornePathogen Training, Public Service License (based upon position). Employees with medical conditions, which prevent active participation in or prohibits certification in any of these required trainings must discuss the matter with their supervisor and submit a request for a waiver.
Assists the Rehabilitation Counselor in developing Individualized Plans by providing information onconsumersprogress, outcomes and participation.
May be responsible for completion and maintenance of any and all consumer records.
Serves as CW,Inc.participant liaison to DDS staff persons, family members or residential providers, and advocated for program participants. May attend team meetings.
Conducts all program activities in accordance with CARF, State and Federal Agencies (DDS, DMHAS, DCF andetc.) and CWemployees.
Must be able to thoroughly supervise those with disabilities

Qualifications and education requirements
College Degree preferred
At least three (3) years in the human services field andatleast one(1) year in a supervisory capacity of professional staff.

The Day Support Options Specialist position can be a physically demanding position. While performing the duties of this job, the incumbent is regularly required to reach using hands or arms. The incumbent must spend the entire time at work standing, walking, lifting heavy objectsetc. The incumbent must frequently lift and/or move up to 50 pounds and occasionally lift and/or more than 70 pounds. Specific vision abilities required by this job include close vision, and the ability to adjust focus. It is important to note that the Day Support Options Specialistmay have to transfer individuals from chairs to wheelchairs.

Must submit to background check prior to hire and ongoing as dictated by contract. Must be able to pass criminal and drug screening processes to meet current contract eligibility requirements that apply.
